1)提供所投设备国家食品药品监督主管部门颁发的《医疗器械注册证》。

2)提供所投眼科手术显微镜设备原厂DATASHEET

3如果投标人所投的货物不是投标人自己制造的,投标人应得到眼科手术显微镜制造商同意其在本次投标中提供该货物的正式授权书(参见第一分册格式IV-9-4)和制造商资格声明(参见第一分册格式IV-9-2);其他投标产品在中标后具有供货能力。

3. Bidder's qualification requirements

 Qualifications or performance of the Bidder:

1) Provide the Medical Device Registration Certificate issued by the national Food and Drug Regulatory department of the invested equipment.

2) Provide the original factory DATASHEET of the invested ophthalmic surgical microscope equipment.

3) If the goods submitted by the Bidder are not manufactured by the Bidder, the Bidder shall obtain the approval of the formal authorization (see sub-volume format IV-9-4) and the manufacturer's qualification statement (see sub-volume format IV-9-2); the other bidding products shall be supplied after winning the bid.
